ABSTRACT

Bowel sounds are one of the important physiological signals of the body,and different bowel sounds can reflect different gastrointestinal states.In this paper,long time bowel sound data is obtained with wearable full belly bowel sound recorder which is independent designed.After adaptive noise cancellation and wavelet threshold denoising,voice endpoint detection method based on short-time energy is used to identify effective bowel sounds.Experiments and results show that the sound recorder is simple and reliable.Through processing,analysis and endpoint detection algorithm,the recognition accuracy of effective bowel sounds is high,which has certain clinical practicality and research significance.

ABSTRACT

Based on the heart rate monitor front end AD8232 chips and Zigbee system solutions CC2530 single chip, the wireless ECG monitor has been realized in order to implement the acquisition, processing and wireless transmission of electrocardiogram signal. Meanwhile, the real-time heart rate has been calculated and wirelessly transmitted. The wireless ECG monitor is characterized by low power consumption, small size and simple operation. It can be seen from the experimental results that the wireless ECG monitor designed in this paper can successfully obtain favorable ECG data and real-time heart rate data and display it.

ABSTRACT

We designed a multi-functional medical wireless gateway based on Zigbee Bluetooth and WiFi Technology to monitor the medical physiological status of subjects in real time. On the one hand, the gateway acts as the Zigbee coordinator, organizing the Zigbee network and controlling multiple medical digital terminals. On the other hand, the gateway implements a heterogeneous wireless network. It establishes the communication between the medical digital terminals and the host computers or hand-held devices. In addition, we developed a display interface based on the LabVIEW platform, implemented the display and storage of various physiological data on the host computer, and verified the availability of the medical wireless gateway.

ABSTRACT

Objective@#To investigate the clinical value of combined detection of serum miR-378 and miR-21 in gastric cancer (GC).@*Methods@#Eighty-seven patients with GC and 78 patients with colorectal cancer(CRC) from National Cancer Center/Cancer Hospital, Chinese Academy of Medical Sciences were selected, 83 individuals undergoing healthy physical examination were selected as the healthy controls. The levels of serum miR-378 and miR-21 were detected by quantitative real-time PCR (RT-qPCR) (result data were transformed as log2 for analysis).@*Results@#Relative expression levels of miR-378 in the serum were -1.24, -3.25 and -2.73 in healthy controls, GC and CRC patients, respectively. Compared with the healthy controls, the levels of serum miR-378 were significantly decreased in GC and CRC patients (both P<0.05). Relative expression levels of miR-21 in the serum were 0.11, 2.34 and 2.47 in healthy controls, GC and CRC patients, respectively. Compared with the healthy controls, the levels of serum miR-21 were significantly up-regulated in GC and CRC patients (both P<0.05). Moreover, the serum level of miR-378 in GC patients was inversely associated with tumor clinical stage (P<0.05). However, the level of miR-21 showed no significant differences among patients with different clinical and pathological characteristics (all P>0.05). The area under the receiver operating characteristic curve (AUC), sensitivity and specificity of miRNA-378 to diagnose GC was 0.770, 82.0% and 66.0%, respectively, and were 0.900, 85.0%, and 88.0% of miR-21, respectively. The AUC, sensitivity and specificity of combined detection of serum miR-378 and miR-21 to diagnose GC were 0.930, 92.0% and 87.0%, respectively, while the AUC of combined detection of serum CEA and CA-199 was 0.767, the AUC of combined all of the four factors was 0.946.@*Conclusion@#The combined detection of serum miR-378 and miR-21 have a certain effect on diagnosis of GC.

ABSTRACT

Objective To design a high performance and low power consumption ECG signal acquisition system which can meet the demand for long time monitoring of the physiological status of patients.Methods The prototype system utilized low power ECG analog front end ADAS1000 and MSP430F5529 microcontroller to achieve configuration of AFE and back-reading of ECG data by SPI bus. Results This system implemented 24-hour dynamic ECG monitoring of patients in active state, and the data acquired were accurate and reliable.Conclusion The system realizes PCB integration, low power consumption, and can be used for battery powered portable application such as wearable devices.

ABSTRACT

To investigate the effect of stepwise paced breathing (PB) on pulse transit time (PTT), we collected physiological signals of electrocardiogram (ECG), respiration and arterial pulse wave during a procedure of stepwise PB, which consists of 6 different breathing rates changing in a protocol of 14.0-12.5-11.0-9.5-8.0-7.0 breath per minute (BPM), with each breathing rate lasting 3 minutes. Twenty two healthy adults involved in this experiment and the change of PTT was analyzed during the stepwise PB procedure. In our study, the PTT was measured by calculating the time interval from the R-spike of the ECG to the peaks of the second derivative of the arterial pulse wave. Ensemble empirical mode decomposition (EEMD) was applied to PTT to decompose the signal into different intrinsic mode function, and respiratory oscillation and trend component (baseline) in PTT were further extracted. It was found that the respiratory oscillations in the PTT increased with decreasing of the PB rate, and many of the subjects (14 out of 22) showed the phenomena of PTT baseline increasing during the stepwise PB procedure. The results indicated that the stepwise PB procedure induced a high level of cardiovascular oscillation and produced an accumulative effect of PTT baseline increase. As PTT is capable of predicting changes in BP over a short period of time, increase of PTT baseline indicates the decrease of blood pressure. The experiments showed that the stepwise PB procedure could reduce blood pressure for most subjects. For future work, it is necessary to develop certain indices differentiating the effectiveness of the stepwise PB procedure on the PTT baseline change, and to test the effectiveness of this stepwise PB procedure on blood pressure reduction for patients with essential hypertension.

ABSTRACT

Objective To investigate the effect of dexmedetomidine on patients with pancreatic cancer treated by high intensity focused ultrasound (HIFU) under general anesthesia.Methods One hundred and twenty ASA Ⅱ or Ⅲ patients with pancreatic cancer undergoing HIFU treatment,were divided into control group (group C) and dexmedetomidine group (group D) by random digits table method,each group with 60 cases.Dexmedetomidine was intravenously infused at 0.2 μ g/(kg· h) until the end of operation after a loading dose of 0.7 μg/kg over 15 min before induction of anesthesia in group D,while the equal volume of normal saline was infused in group C.Anesthesia was induced with intravenous injection of fentanyl 2-3 μg/kg and propofol 1.5-2.0 mg/kg and cisatracurium 0.2 mg/kg at 5 min after administration of the loading dose.Anesthesia was maintained with sevoflurane,remifentanil,propofol,cisatracurium.The propofol,remifentanil and sevoflurane consumption,the incidence of postoperative agitation,delirium,delayed recovery were recorded.Results The intraoperative heart rate in group D was significantly lower than that in group C (P < 0.05).The propofol,remifentanil and sevoflurane consumption in group D were significantly shorter than those in group C [(341.4 ± 45.7) mg vs.(520.5 ± 50.8) mg,(1.7 ± 0.5) mg vs.(2.3 ± 0.8) mg,(11.6 ± 4.1) ml vs.(16.7 ± 3.8) ml,P < 0.05].The incidence of postoperative agitation and delirium in group D were significantly lower than those in group C [6.7%(4/60) vs.21.7%(13/60),8.3% (5/60) vs.26.7% (16/60),P < 0.05].There was no statistically significant difference in the incidence of delayed recovery between group D and group C [3.3% (2/60) vs.1.7% (1/60),P > 0.05].Conclusions Dexmedetomidine given HIFU treatment of pancreatic cancer patients can reduce the amount of general anesthetic,reduce the incidence of postoperative agitation and delirium,intraoperative heart rate.

ABSTRACT

To investigate the modulation effects of breathing movement on cardiovascular system and to study the physiological coupling relationship between respiration and cardiovascular system, we designed a comprehensive testing system for cardiorespiratory interaction research. This system, comprising three parts, i. e. physiological signal conditioning unit, data acquisition and USB medical isolation unit, and a PC based program, can acquire multiple physiological data such as respiratory flow, rib cage and abdomen movement, electrocardiograph, artery pulse wave, cardiac sounds, skin temperature, and electromyography simultaneously under certain experimental protocols. Furthermore this system can be used in research on short-term cardiovascular variability by paced breathing. Preliminary experiments showed that this system could accurately record rib cage and abdomen movement under very low breathing rate, using respiratory inductive plethysmography to acquire respiration signal in direct-current coupling mode. After calibration, this system can be used to estimate ventilation non-intrusively and correctly. The PC based program can generate audio and visual biofeedback signal, and guide the volunteers to perform a slow and regular breathing. An experiment on healthy volunteers showed that this system was able to guide the volunteers to do slow breathing effectively and simultaneously record multiple physiological data during the experiments. Signal processing techniques were used for off-line data analysis, such as non-invasive ventilation calibration, QRS complex wave detection, and respiratory sinus arrhythmia and pulse wave transit time calculation. The experiment result showed that the modulation effect on RR interval, respiratory sinus arrhythmia (RSA), pulse wave transit time (PWTT) by respiration would get stronger with the going of the slow and regular breathing.

ABSTRACT

We evaluated the anti-HBV effects of nucleotide analogues, Entecavir (ETV) and Lamivudine (LAM) targeting mouse model of HBV persistent infection with recombinant adeno-associated virus 8 carrying 1.3 copies of HBV genome (rAAV8-1.3HBV). Ninety percent (27 of 30 mice) of rAAVS-treated mice were chosen as mouse model. Four groups were orally administrated with different doses of ETV (1 mg/(kgd) or 0.1 mg/(kgd)) and LAM (500 mg/(kgd) or 100 mg/(kgd)) once a day for 10 days. The other two groups were set as normal saline treated and untreated control. We detected the levels of HBV DNA, HBeAg and HBsAg in sera at different time. Results indicate that HBV DNA level decreased significantly (P < 0.05) in drug-treated groups compared with normal saline group after drug administration. Fifteen days after the drug withdrawal, HBV DNA level rebounded back obviously (P < 0.05) in groups with low doses of ETV and LAM. However, there was no apparent change of HBeAg and HBsAg in the whole process among all groups. These results showed that our model could reflect the anti-viral effect of nucleotide analogues. This model can be a useful and convenient tool for anti-HBV drug discovery.

Based on CC2530 SoC and Micro-SD card, the system consists of three acquisition nodes and a coordinator node. The former three nodes implement data collection and wireless transmission by collecting the data of blood pressure, body temperature and ECG, while the last one fulfills the tasks of fast network organization, data reception and assorted storage.

By combining with informatics theory, ta system model consisting of feature selection which is based on redundancy and correlation is presented to develop disease classification research with five gene data set (NCI, Lymphoma, Lung, Leukemia, Colon). The result indicates that this modeling method can not only reduce data management computation amount, but also help confirming amount of features, further more improve classification accuracy, and the application of this model has a bright foreground in fields of disease analysis and individual treatment project establishment.

ABSTRACT

Objective To explore the functional orientation of township hospitals in developed areas to achieve goals of the ongoing health reform.Methods Two rounds of Delphi consultation was made among 18 experts from around the country to learn the degree of importance of every function.Results The experts' average seniority of service in their fields was 23.28±10.41 years.The positivity coefficients of the two-round consultation were 100%.Kendall' S coefficients of the two-round consultation were 0.226 and 0.265 (p<0.01).These data are of statistical importance,suggesting consensus among these experts and the consultation meeting the needs of forecast.In these consultation,seven class-1 functions and and 27 class-2 functions were determined.Medical services and public health services account for similar proportion of work of township hospitals.Conclusion The functional orientation was feasible and forward looking,for reference of township hospitals in developed areas.

ABSTRACT

Objective To investigate the therapeutic effect and the immune function impacts of high intensity focused ultrasound(HIFU) for the treatment of hepatocellular carcinoma.Methods Twenty-five cases with primary liver cancer and 6 cases with metastatic liver cancer were treated with HIFU.The clinical symptoms,AFP levels,color Doppler ultrasound detection,CT and changes of T-cell subsets ( CD3 +,CD4 +,CD8 +,CD4 +/CD8 + ) were recorded before and after treatment.Results After HIFU treatment,among 31 patients,the clinical remission rate was 89.7% ( 25/29 ).AFP was decreased in 83.3% ( 15/18 ) of the patients.Color Doppler showed reduced tumor sizes and reduced or disappeared blood supply in most of the patients.CT scan displayed that the density of the target region was decreased and necrosis of the lesions occurred.The percentage of CD3 + and CD4 + cells and the ratio of CD4 +/CD8 + were significantly increased after a period of treatment( P <0.05 ).The proportion of CD8 + cells was decreased significantly after a period of treatment ( P < 0.05 ).Conclusion HIFU is an effective technique for the treatment of hepatocellular carcinoma.Meanwhile,it can improve human immune function to certain extend.

ABSTRACT

Determining the approach to spread the knowledge and technology for preventing birth defects using the diffusion of innovations theory.Identifying the target group,orientation messages and promotion guidelines based on related literature,laws and clinical epidemiology studies.Pathways integrating both inter-personal communication and IT have pushed the adoption percentage of innovative knowledge for preventing birth defects up from 0% to 97 % in almost no time,helping the region ranking the first in China.The innovation diffusion model has caught attention of both the government and all the community.Prevention measures against birth defects are accepted community wide,while medical workers are regulated by law in their behaviors of mothers and children healthcare services to set a model for prevention of other diseases.
